Ready to enroll?
Use this form to start your child(ren)’s enrollment process.
Notes: After you enter your information on this page and press ENROLL, you will be directed to a separate enrollment form. You are able to enroll multiple children using this same enrollment form. Once you finish with one child, you will see an option to “add a patient”. Do this for each child you are enrolling. Enter the information for your child (not for you).

At the end of the enrollment form, you will be asked to provide your credit card information. This puts your card on file. Upon enrollment confirmation by our office, it will be used to charge your one-time family enrollment fee of $125. This will hold your child(ren)’s spot in the practice. You will be able to choose how your subscription is charged moving forward.
